PLEASE READ CAREFULLY 

This waiver applies to any guest under 19 years of age or to minors attending the park without a parent or guardian present.

A parent or legal guardian must complete the electronic waiver and include the minor’s information, even if the parent or guardian will not be present at the park.

In addition to the electronic waiver, a printed waiver must be completed and notarized. A new notarized waiver is required EVERY time the minor attends the park without a parent or guardian present. The notarized waiver must be presented at check-in.

All minors 18 years of age or younger must have:

• An electronic waiver on file completed by a parent or guardian
• A photo of the parent or guardian’s identification uploaded through the electronic waiver process

If a minor is attending without a parent or guardian present, they must:

• Be accompanied by a responsible adult 19 years of age or older
• Bring a notarized waiver specifying the responsible adult that they are attending the park with and the dates of attendance
• Completed notarized waivers will be saved in the minors waiver account each time they are recieved by the Indian Mountain Staff
